Bob began his career in the insurance industry in 1998 with Aflac. He worked his way up through the ranks as an agent to then District and Regional Sales Coordinator roles overseeing a large portfolio of accounts and agents.
In 2015, he set out on his own and established Black Rok Benefit Alliance, LLC that specialized in all group business. In 2018, he moved on to establish Tier#1benefits, where he incorporated Group Voluntary benefits into his portfolio. In August 2020 he became part of the Acrisure family as an Agency Partner.
Bob has built a solid rapport within the industry and maintained his growing list of clients by always making his policyholders' and clients' needs his utmost priority.
He currently resides near Pittsburgh, PA with his wife and family that now includes 7 grandchildren. He enjoys spending his spare time at his cabin in the mountains and enjoying quality time with his family.

Brian is a 2008 graduate of the University of Pittsburgh at Johnstown, where he majored in Business Marketing and played baseball for the Pitt Mountain Cats.
Brian began his insurance career in 2008, as an Independent Sales Contractor for Aflac. This career choice was a perfect match for him, as he is very passionate about helping people and enjoys educating his clients about the products available that will match their needs.
In 2021, Brian joined Acrisure-Tier1#1benefits, which allowed him to offer a wider range of products with a larger variety of leading companies. His knowledge and experience has made him an essential member of our team.
Brian currently resides in South Fork, PA with his wife and daughter. He enjoys spending his free time coaching youth sports and volunteering in the community.
HOW DO I FILE A CLAIM?
1. BEAZLEY (GAP/GLI):
Claim forms:
No claim forms required.
*Please see your brochure on what's covered. If you need a copy, email us at [email protected]. Also, please see directions under the "Company-Specific FAQS" section to the right of this page for the recommended option to set up an account in the employee portal to file claims and check statuses on Beazley GAP/GLI Claims online.
What's needed:
Depending on the nature of the claim, you will need to provide supporting documentation, such as: ER Report, UB-04 for confinements/admissions, Operative Report for surgeries, After-Care Summaries for physician visits, Radiology Benefits for x-rays, Ambulance Trip Sheets if transported by ambulance, etc. Please provide your ID# on your submission that was provided to you on your card.
*All of these can be obtained through the billing department or medical records department at the facility in which you were treated.
How do I file GAP/GLI claims:
All Beazley claims can be emailed to [email protected], or fax your claim to 1-813-289-7937, OR submit online under the employee portal. *Please see instructions under the Company-Specific FAQS section on setting up an online account under the employee portal. From here, you can download claim forms, see instructions on how to file and check the status of existing claims.
How do I know if my GAP/GLI claim was processed:
To check the status of a GLI/GAP claim that's already been filed, please call 877-503-7064 Option 4 & Option 1, OR again, login to the employee portal site (directions in column to the right).
2. BEAZLEY/GLOBE LIFE- LOOMIS

What's needed:
You will need to obtain a copy of any EOB's (Explanation of Benefits) related to your claim, Itemized Physician/Hospital Bills (specifically request a UB-04 form) and ensure that it has diagnosis codes, procedure codes, billed amounts, dates of service, provider's name, provider's Tax ID# & NPI # (National Provider Identifier #) listed on the form to avoid a delay in processing. *Important: Please specify claim checks should be sent to you, the policyholder directly and not the provider when submitting UB-04 forms, as the billing department will sometimes inadvertently check the box for claim checks to be sent to them.

COMPANY-SPECIFIC FAQS
Directions for registering an account on the online portal with Beazley:
A: Go to http://www.beazleybenefits.com and click on Insured Registration. This will enable you to register with Beazley and to create a username and password for access to our Employee Portal. To do so, you will need a Group number (from your employer) as well as your own Social Security number (last four digits), birthday and zip code. If you need assistance, contact 1-877-503-7064.
A: On the portal, you will be able to view your coverage details, file claims and check claim's status, retrieve important documents and access materials and forms from our Resource Center.
A: To view coverages, go to the Coverages section of the Employee Portal. There, you can view an overview of your coverages with Beazley, as well as a detailed Schedule of Benefits. The first time you access the Employee portal, you will retrieve your Certificate of Coverage from the eDelivery section of the Employee Portal. Print this important legal document and keep it on file as proof of your coverage.
A: To file a Claim, go to the Resource Center section of the Employee Portal. There, you can download claim form with instructions for filing. To fax a claim, submit to 1-813-289-7937. To check the status of a claim, or a view your claim history, go to the Claims section of the Employee portal. There, you can view details about claims that are being processed or have been paid. If you have questions about a claim, contact 1-877-503-7064.
